Who we are

What this app is
The planner helps a clinic owner or manager see an example revenue estimate for adding FOCUS Shockwave. It can also pass along a request for a demo or a callback. It is for business planning. It is not a patient service and it does not give medical advice.
What we collect
You can view an estimate without sending us anything. We receive business contact details only if you ask MODRN to contact you:
- Your name
- Practice or company name
- Email address
- Practice type
- Phone number, if you choose to give one
- US state, if you choose to give one
- Best time to reach you, if you choose to give one
- The estimate inputs and example results you choose to send with that request
We do not ask for patient information. Please do not send patient names, health information, or anything about a patient's care. This version does not collect text-message consent. Contact is by email or phone only.
We do not collect payment card numbers, government ID numbers, or precise location.
How we use it
We use these details to respond about clinic equipment and the estimate you asked for, and to schedule a demo if you book one. We do not use them to send text messages in this version.
Where it is stored
If you tap Send, we store the inquiry in Less Annoying CRM, the customer system BBC LLC d/b/a MODRN Vitality uses. A new contact is created with your name, practice, email, and any phone or state you provided. Practice type, timing, and the example estimate are stored in a note on that contact.
If the same email is already in the CRM, we add a note to that existing contact. We do not replace or delete the existing contact.
When a CRM group named ChatGPT is available, the contact is added to that group so the team can see the inquiry came from this app. If that group cannot be attached, the note still says the inquiry came from the ChatGPT app.
Saving a practice name may also create a company record in the CRM when that company is not already there. That is how the CRM stores the practice name. This app does not edit or delete company records.
Demo booking
The demo button opens a Calendly page. Calendly handles that booking under its own terms. BBC LLC d/b/a MODRN Vitality sees the booking in its Calendly account.
ChatGPT and hosting
The planner runs inside ChatGPT. OpenAI processes the conversation under its own terms and privacy policy. We receive the business details submitted through the planner, not your full chat history. The app server is hosted on Vercel so it can answer ChatGPT. Vercel processes technical connection data as needed to run the site.
Who we share it with
We do not sell your information. We share the inquiry with Less Annoying CRM so we can keep it, with Calendly if you book a demo, and with the hosting provider that runs this app. People at BBC LLC d/b/a MODRN Vitality who respond to equipment inquiries can see it.
United States only
This version of the planner is for clinics in the United States.
How long we keep it
Business inquiries are kept so MODRN can respond and can keep ordinary business records. Email alexandria@modrnemage.com to ask what we stored or to request deletion.
Your choices
The estimate works without a callback. Booking a demo and asking for a call are optional. You can close the form without sending it.
Children
This app is for clinic businesses. It is not directed to children.
Changes
We may update this policy. The effective date at the top of this page is the date of the version in effect.
